Academic Transcript Information
How to request an official copy of your academic transcript from the Office of the Registrar
- Obtain a Transcript Request Form from the Office of the Registrar or web site.
- Complete a separate Form for each address your transcript is to be mailed to.
- Mail or Fax the request form (address and phone numbers are listed under Request options at the bottom of this page). On-line submittal of this form is not currently an option. E-mail and telephone requests are not currently accepted, due to the requirement of a signature.
- Transcript requests will not be processed without a signature.
Official transcripts are not issued until all accounts with the university are paid in full.

- Previous Name (s)
- Social Security Number
- Date of Birth
- Current Address and Phone Number
- Day-time Phone Number
- Dates of Attendance
- Graduation Date
- Address Where Transcript is to Be Sent
- Student Signature (required)
- Number of Copies needed
* A limit of ten per calendar year will be issued at no charge.
A $5.00 fee per transcript will be charged for requests over ten per calendar year.
There will be a $10.00 fee for faxed transcripts. Faxed copies are unofficial.

Transcripts sent to the student will be stamped "Issued to Student". Upon receipt of the completed form (s), official transcripts are normally mailed within twenty-four hours. However, during peak periods (i.e. commencement, registration and examinations) there will be some delay. Therefore, transcripts should be requested well in advance of these periods. The Office of the Registrar makes every effort to expedite your transcripts to your intended recipients.
