• Application Forms

    Students who have been diagnosed with a disability are encouraged to apply for accommodation through the Office of Disability Services. Please plan to submit your application and supporting materials at least 30 days prior to the initial semester of enrollment to allow adequate time for the coordination of reasonable accommodations. Please allow 15 working days for personnel to process your application, review the documentation, and contact you. If additional information is needed, it will likely take longer to process your application.

    Instructions For Applying For Disability Services:

    1. Apply and be accepted to Hardin Simmons University.
    2. Review the documentation criteria and have your physician or clinician fill out and submit the appropriate form (found below).
    3. Fill out the student application form.
    4. Meet with the Disability Coordinator to discuss approved accommodations, if any.

    Application for Short-Term Disability Services:

    In the event of an injury, surgery, or other documented short-term disability, you may apply for reasonable accommodations using the Application for Temporary Disability Support Services.
