ASMF Application

All information is required. Enrollment is accepted on a first come first-served basis. Please make your check or money order payable to ASMF and mail the $50 non-refundable registration fee (which will be applied to the total fee) to the address on the following page. Scholarship application deadline is May 4th, 2012. Total fee is due by June 1st, 2012. Late fee of $50 assessed after June 1st, 2012.

*All campers must choose one Elective; campers may choose to participate in either or both Additional Activities (dependent upon age/instrument). Campers in the ASMF Sinfonietta (beginning orchestra) will participate in assigned music classes rather than Electives but may still register for Private Lessons (subject to availability).
